Fulton County Job & Family Services is Requesting Qualification for a Head Lice Case Management Program with consumers referred from Fulton County Job and Family Services. FCJFS wishes to contract with an experienced community consultant for assistance in the design and implementation of providing Head Lice Case Management Program Services as follows:

The Head Lice Case Management Program Services shall be comprised of services that include:

• Assessment of head lice infestations in student populations or children residing in homes with student populations.
• Treatment to eradicate infestations in student populations or children residing in homes with student populations.
• Education for families and students or children residing in homes with students to minimize infestation problems in-home visits,

The goal of the Head Lice Case Management Program is to decrease absenteeism from school and improve family health in Fulton County, Ohio. Head Lice Case Management Program Services will be provided in the Fulton County Schools and the consumer’s homes.

This contract is contingent upon the availability of funding. In the discretion of the agency, should funding not be available for the proposed services, this contract may be terminated or reduced as determined necessary by the agency.
